Submit Backup Requests and Major Incidents via ServiceNow (Back-Office Users)


The Office of Information Technology has moved the ability to submit backup requests and major incidents into ServiceNow.

Note: These forms are only available to those with an ITIL and Business stakeholder license to ServiceNow


How to access the Major Incident Request form in ServiceNow

1. Log in to ServiceNow (
2. Select Major Incident – Create New Major Incident  (left navigation menu)

How to access the Backup Request form in Service Now

1. Access the Tech Help Portal via > Tech Help page

2. Select Service Catalog

3. Select IT Internal from the Service Catalog Categories menu (lower left)

4. Select Backup Request/Dec option
